Source text - Turkish Günümüzde prevalansı hızla artmakta olan obezite, tip 2 diabet, kardiyovasküler hastalıklar, hipertansiyon ve bu hastalıkların birlikte bulunduğu metabolik sendrom ile yakından ilişkilidir. Adipoz doku enerji depolama görevine ek olarak, dolaşımdaki adipokinlerin sekresyonu aracılığı ile enerji metabolizmasını düzenler. İnsülin direnci, obezite ile ilgili bozukluklara zemin hazırlayan faktörlerin başında yer alır (1). Hücresel glukoz uptake’ini sitimule eden ve antilipolitik bir hormon olan insülin, adipoz doku üzerine direkt etkisi ile ve hipoglisemi aracılığıyla iştahı etkileyerek kilo alımına yol açabilir. Adipozitlerden sentezlenen leptin, hipotalamusta doyma merkezindeki leptin reseptörlerini etkileyerek iştah ve vücut ağırlığını düzenler. Leptin verilmesi besin alımını ve vücut ağırlığını azaltır. Bununla birlikte, obez insanlarda hipotalamusta leptin direnci ile uyumlu olarak iştah ve ağırlık artışı olduğuna inanılır. Bundan başka, insülinin adipozitlerden leptin üretimini uyardığı ve bunun tersine, leptinin pankreatik beta hücre fonksiyonunu insülin sekresyonu üzerine hem stimulatör hem de inhibitör etkilerle etkileyebileceği bilinmektedir. Plazma leptin düzeyleri beden kütle indeksi (BKİ) ve vücut yağ oranı ile belirgin ilişkilidir.
Adiponektin, insülin duyarlılığının düzenlenmesinde önemli rolü olan bir hormon olup, obezite ve tip 2 diabette kan konsantrasyonları azalır (2). Adiponektin lipid metabolizması, glikoz metabolizması ve insülin direnci ile ilişkilidir (3,4). Hayvanlara rekombinant adiponektin verilmesi kasta glikoz tutulumu ve yağ oksidasyonunu artırır, karaciğerde yağ asidi tutulumunu ve hepatik glikoz üretimini azaltır, ve tüm vücudun insülin direncini düzeltir (5). Bundan başka, adiponektin bir anti-inflamatuar faktör olarak rol oynar ve ateroskleroz, hipertansiyon ve koroner kalp hastalığı riskinin azalmasına katkıda bulunur (6,7). Aşırı vücut yağı, özellikle abdominal visseral yağ birikimine sıklıkla hipoadiponektinemi eşlik eder; ki bu durum, obezite ve aterosklerotik vasküler hastalıklar arasındaki ilişkinin bilinen bir mediatörüdür (8).
Obezlerde hiperkolesterolemi, yüksek trigliserid (TG) ve LDL kolesterol düzeyleri ile düşük HDL kolesterol düzeyleri sık rastlanan bir özelliktir. Çeşitli çalışmalar, obezlerde oksidatif stresin arttığını ve sağlıklı kontrollere kıyasla LDL’nin lipid peroksidasyonuna duyarlılıklarının daha fazla olduğunu göstermiştir (9,10). Oksidatif hasarın artmasının antioksidant kapasitenin azalması ile uyumlu olabileceği öne sürülmektedir. Obezitede LDL oksidasyonunun, HDL’nin antioksidant savunmalarındaki bozulma ile ilişkili olduğu bulunmuştur; ki burada, HDL ile ilişkili enzimler olan paraoksanaz (HDL-PON) ve lesitin kolesterol asil transferaz aktivitesindeki azalmalar sorumlu tutulmaktadır (11). HDL-PON’un enzimatik aktivitesi sağlıklı insanlar arasında geniş farklılık gösterir. PON aktivitesi düşük olanlarda oksidatif hasar ve lipid peroksidasyonu ile ilişkili hastalık gelişmesi riskinin, PON aktivitesi yüksek olanlara kıyasla daha büyük olabileceği öne sürülmüştür (12).
Psikiyatrik hastalıkların ilaçla tedavisi süresince sıklıkla iştah artışının eşlik ettiği vücut ağırlığında bir artış meydana gelir. Oluşumunu ve zamanını tahmin etmenin güç olduğu bu yan etki, sonunda obeziteye yol açar ve hastaların bir kısmında etkili olmasına rağmen tedavinin kesilmesine neden olur. Obezite, pek çok kronik hastalığın gelişmesi riskini de artırır. Obez hastalar koroner arter hastalığı, hipertansiyon, hiperlipidemi, diabet, bazı kanser türleri, serebrovasküler olaylar, osteoartritler, pulmoner hastalıklar ve uyku apnesi gibi hastalıklar yönünden daha fazla risk taşırlar (13-15). Bu nedenle, birçok kronik hastalıktan korunmada veya oluşmuş bir hastalığın prognozunu iyileştirmede kilonun azaltılması anahtar rolü oynar.
Bu çalışmada, psikiyatrik ilaçlarla tedavi edilmekte olan yetişkin kadınların vücut ağırlığı, bileşimi ve biyokimyası ile leptin, adiponektin ve paraoksanaz düzeylerinde oluşan değişikliklerin metabolik ve kardiyovasküler risk faktörleri ile ilişkisinin araştırılması amaçlanmıştır.
TARTIŞMA
Birçok hastalığın ilaçla tedavisi süresince vücut ağırlığının artması, hastalığın iyileşmesinin bir belirtisi olarak kabul edilir. Bununla birlikte, vücutta iştahı düzenleyen mekanizmalardaki ilaçların yol açtığı bu değişiklikler, aşırı kilo artışı ile sonuçlanabilir ve böylece, önerilen medikal tedaviye uyumu tehlikeye sokabilir. Pek çok ilaç, terapötik etkilerinin olumsuz bir sonucu olarak vücut ağırlığını değiştirebilecek kapasitededir (16). Bu grupta antipsikotikler, antidepressantlar ve duygu-durum düzenleyicileri gibi psikotropik ilaçlar önemli yer tutmaktadır.
Adipoz dokuda aşırı yağ birikimi ve bu yağın abdominal dağılımı insan sağlığını olumsuz yönde etkiler. Abdominal obezite, glukoz, insülin ve lipid metabolizmasındaki bozukluklarla güçlü bir şekilde ilişkilidir (17-20). Adipoz doku enerji depolama görevine ilaveten, biyolojik olarak aktif pek çok adipokinler salgılar. Bunlardan leptin ve adiponektinin insülin duyarlılığında önemli rolü olduğu bilinmektedir.
Leptin, başlıca enerji homeostazı ve tokluk ile ilişkili olup, adipoz doku dışında hipotalamus, hipofiz, plasenta, iskelet kasları, mide ve süt bezlerinde üretilen bir proteindir (2). Dolaşımdaki leptin düzeyleri yağ kütlesi ile orantılı olarak artar ve dolaşan leptin, adipoz dokuda depolanan enerji miktarı hakkında hipotalamusa bilgi taşır. Nadir genetik mutasyonlar insanlarda leptin veya leptin-reseptör eksikliğine neden olur. Leptin eksikliği nedeniyle aşırı obez olan çocuklara leptin verilmesi, enerji alımlarında ve yağ kütlelerinde belirgin azalma sağlar (21). Bununla birlikte, obez insanlara ekzojen leptin verilmesinin adipoziteyi azaltmada yetersiz kalması, obezlerde artmış adipozite leptin içeriği ve dolaşımdaki yüksek leptin düzeylerine karşı bir leptin direnci durumunu yansıtmaktadır. Obez insanlarda leptin direncinin leptinin beyine taşınmasında bir bozukluk ve/veya hipotalamik leptin sinyalinin azalması ile ilgili olduğu düşünülmektedir (2).
Kilo artışı birçok psikiyatrik ilaç tedavisinin yol açtığı bir yan etkidir. Kilo artışına ilaçların sedatif etkisi nedeniyle fiziksel aktivitenin azalması ve yüksek kalorili içeceklerin fazla tüketilmesinin katkıda bulunduğu düşünülür. İnsülin ve leptin vücut ağırlığının düzenlenmesinde rol alır. Bir çalışmada (22), farklı antipsikotiklerle tedavi edilen hastalarda bu hormonların düzeyleri araştırılmıştır. BKİ yönünden benzer olmalarına rağmen, olanzapine alan grubun konvansiyonel antipsikotik alanlardan daha yüksek insülin düzeylerine sahip olduğu bulunmuş, bu durum olanzapinin insülin sekresyonu üzerine olası etkisine bağlanmıştır. Olanzapine ve clozapine tedavisi süresince leptin regülasyonunun da değiştiği belirlenmiş ve bu ilaçların insülin ve leptin düzeylerine etkisinin kilo alımını indükleme özellikleri ile ilişkili olabileceği sonucuna varılmıştır.
Çalışmamızda çeşitli psikotrop ilaçlarla tedavi edilmekte olan hastaların glikoz, insülin, HOMA-R, leptin, trigliserid, total ve LDL kolesterol düzeyleri, aynı BKİ grubunda (BKİ >30.00 kg/m2) olmalarına rağmen kontrol grubundan anlamlı olarak daha yüksek bulunmuştur. Ayrıca vücut yağ kütlesi (%) ile bel ve kalça çevresi ölçümleri çalışma grubunda anlamlı olarak daha yüksektir. Çalışma grubunda, karaciğer enzimlerinin kontrol grubundan anlamlı olarak daha yüksek olmasında, obezitelerinin yol açtığı bozukluklarla birlikte kullanılan ilaçların da katkısı olduğu düşünülmüştür. Bu durum, psikotrop ilaçların vücut ağırlığı ile ilgili hormonları etkileyerek, metabolik ve kardiyovasküler bozuklukların oluşumuna zemin hazırlayabileceğine işaret etmektedir.
Psikotropik ilaç tedavisinin yol açtığı kilo alımının, leptin düzeyinde de bir artışla ilişkili olabileceği öne sürülmüştür. Şizofreninin clozapine ile akut tedavisi sırasında serum leptini, tedavinin başlatılmasından iki hafta sonra 2 kat artmış ve ilk 10 hafta süresince yüksek kalmıştır. Bu sürede vücut ağırlığı ortalama 4.2 kg artış göstermiştir (23). Clozapine veya olanzapine ile 4 hafta tedavi edilen hastalarda, artmış leptin sekresyonu ile uyumlu olarak sırasıyla 2.3 ve 3.9 kg alımı saptanmıştır. Haloperidol ile tedavi edilen veya psikotropik ilaç kullanmayan hastaların kilo almadıkları ve sabit leptin düzeylerine sahip oldukları bulunmuştur (24).
Metabolik sendromun şizofreni tedavisi alan hastalar arasında oldukça yaygın olduğu bildirilmiştir (25). Bu durum, kardiyovasküler ve metabolik bozukluklar yönünden önemli risk artışına işaret eder. Bu nedenle, antipsikotiklerle tedavi edilen hastalarda metabolik sendromla ilişkili risklerin varlığının başlangıçta değerlendirilmesi ve izlenmesinin klinik tedavinin bir parçası olması gerektiği önemle vurgulanmaktadır.
Longitudinal bir çalışmada (26), akut olanzapine tedavisinin 8 haftasından sonra plazma leptininde anlamlı bir artışla ilişkili olarak, vücut ağırlığı 3.3 kg artmıştır. Lityum monoterapisinin 8 haftası içinde, 8 kadın ve 7 erkek yatan hastada vücut ağırlığı ortalama 5.9 kg artarken, bu sürede leptin düzeyleri bazalde 6.9 ng/ml’den 10.3 ng/ml’ye anlamlı olarak yükselmiştir (27). Uzun dönem valproik asit tedavisi altında olan epileptik hastalarda tartı alımı, BKI’deki artışla uyumlu olarak plazma leptininde bir artışla ilişkili bulunmuştur (28).
Trisiklik ilaçlar veya paroxetine ile tedavinin 6 haftası süresince vücut ağırlığı sırasıyla 3.5 ve 1.0 kg artarken, farmakoterapi almayan depresif hasta grubunda değişiklik olmamıştır. Plazma leptin düzeyleri ise grupların hiç birisinde değişmemiştir (29). Bir çalışmada, majör depresyon nedeniyle mirtazapine tedavisinin 4 haftasında 2.4 kg alan 11 hastada, bazale kıyasla plazma leptininde sadece çok küçük bir artış bulunmuştur (Psik ilaçlar-1’de 41).
Atipik antipsikotikler quetiapine, olanzapine, risperidone ve clozapine’in leptin ve trigliserid düzeyleri ile vücut ağırlığına etkilerinin araştırıldığı bir karşılaştırmalı çalışmada (30); olanzapine ve clozapine vücut ağırlığı ile trigliserid ve leptin düzeylerinde belirgin bir artışa neden olmuştur. Bu değişkenlerdeki artış quetiapine alanlarda orta düzeyde, risperidone alanlarda ise minimal bulunmuş; ayrıca leptinin BKİ ve trigliserid düzeyleri ile pozitif korelasyon gösterdiği belirlenmiştir.
Çalışmamızda, leptin her iki grupta da kilo, BKİ, bel ve kalça çevresi ile anlamlı pozitif korelasyon gösterirken; adiponektin/leptin oranı ile anlamlı negatif korelasyon göstermiştir.
Uzun dönem (0.5-8.3 yıl; ort:2.51.9 yıl) risperidone monoterapisi ile tedavi edilen şizofreni hastalarında glikoz ve lipid metabolizması ile beslenme kontrolünde rolü olan parametreler araştırılmıştır (31). Vücut yağ yüzdesi, BKİ, açlık kan glikozu, trigliserid, leptin, ghrelin ve prolaktin düzeyleri risperidone tedavisi alan hastalarda sağlıklı kontrollerden anlamlı olarak daha yüksek, HDL kolesterol düzeyleri ise anlamlı olarak daha düşük bulunmuştur. Plazma ghrelin düzeyinin yüksek olmasının iştahı artırabileceği ve böylece metabolik bozulmayı şiddetlendirebileceği sonucuna varılmıştır.
İnsülin duyarlılığını artıran bir hormon olarak çalışan adiponektinin kan konsantrasyonu obezite ve tip 2 diabette azalmıştır (2). Kemirgenlere rekombinant adiponektin verilmesi kaslarda glikoz tutulumu ve yağ oksidasyonunu artırırken, karaciğerde yağ asidi tutulumu ve hepatik glikoz üretimini azaltır ve tüm vücudun insülin direncini düzeltir (5). İnsanlarda adiponektinin vücut ağırlığı, vücut yağ kütlesi ve insülin düzeyleri ile negatif korelasyon gösterdiği bilinmektedir (2).
Adiponektin lipid metabolizması, glikoz metabolizması ve insülin direnci ile ilişkisinden başka, bir anti-inflamatuar faktör olarak da rol oynar (3,32,33). Böylece, obezite ile ilişkili ateroskleroz, hipertansiyon ve koroner kalp hastalığı gibi diğer bozuklukların gelişmesine de katkıda bulunabilir (6,33,34). Aşırı vücut yağı, özellikle abdominal visseral yağ birikimi sıklıkla hipoadiponektinemi ile ilişkilidir, ki bu durum obezite ile aterosklerotik vasküler bozukluklar arasındaki ilişkinin bilinen bir mediatörüdür (8).
Obezitesi olan ve olmayan yetişkinlerde plazma adiponektin, leptin ve rezistin düzeyleri ve bu adipokinlerin insülin direnci ile ilişkisinin araştırıldığı bir çalışmada (35); insülin direnci leptin düzeyleri ile çok güçlü korelasyon göstermiştir. Adiponektin düzeyleri obezlerde, normallere göre anlamlı olarak daha düşük olmakla birlikte, HOMA-R ile anlamlı korelasyonu bulunamamıştır.
Çalışmamızda psikotrop ilaç kullanan grubun ortalama adiponektin düzeyleri, kontrol grubundan anlamlı olarak daha düşüktür (Tablo-2). Hastalar çalışmaya alınırken BKI yönünden obez (>30 kg/m2) olmalarına dikkat edilmesine rağmen, gruplar arasında BKI yönünden anlamlı farklılık bulunmuştur. Bunun üzerine gruplar, yaş ve BKI yönünden ayarlanarak bazı parametrelerin farklılığı tekrar değerlendirilmiştir. Yaş= 38.67 yıl, BKI= 33.54 kg/m2’ye ayarlandığında adiponektin, ilaç tedavisi alanlarda 5.838 0.709 µg/ml ve kontrol grubunda ise 8.804 0.877 µg/ml olup, farklılık anlamlı (p=0.017) bulunmuştur.
Farklı yaş ve obezite derecelerinde olan yetişkin kadınlarda plazma adiponektin ve leptin düzeyleri ile vücut kompozisyonu ve glikoz kullanımı araştırılmıştır (36). Çalışmada, plazma adiponektin düzeylerinin yaş ile değişmediği, fakat düzeylerin vücut yağ yüzdesi, visseral yağ, subkutan abdominal yağ, insülin ve leptin ile negatif yönde ilişkili olduğu bulunmuştur. Ayrıca, adiponektinin glikoz kullanımı ile pozitif ilişkili olduğu belirlenmiştir.
Çalışmamızda, ilaç alanlarda adiponektin düzeylerinin yaş ile anlamlı pozitif korelasyon gösterdiği (r=0.525, p=0.002) bulunmuştur. Ayrıca, ilaç alanlarda adiponektin; glikoz, HDL, folat ve adiponektin/leptin oranı ile anlamlı pozitif, fibrinojen ile anlamlı negatif ilişki göstermiştir. Kontrol grubunda ise adiponektin; PON, adionektin/leptin oranı ve B12 vitamini ile anlamlı pozitif ilişki göstermiştir. Bu sonuçların, adiponektinin kardiovasküler hastalıklar ile ilişkisini doğrular nitelikte olduğu düşünülmüştür.
Hipoadiponektineminin diğer inflamatuar belirteçlerden daha güçlü olarak diabet ve metabolik sendrom ile ilişkili olduğu bildirilmiştir (37). Azalmış plazma adiponektin düzeyleri kardiovasküler hastalık ve hipertansiyon gibi sıklıkla insülin direnci ile ilişkili durumlarda da görülmektedir (38,39).
Plazma adiponektin düzeyi pek çok faktörden etkilenir; bunlar arasında cinsiyet, yaşlanma ve yaşam tarzı sayılabilir. Bazı diyetsel faktörlerin; soya proteini, balık yağları ve linoleik asit gibi, plazma adiponektin düzeylerini artırdığı öne sürülmüştür (37). Bu durum, bu faktörlerin alınmasının diabet gelişmesine karşı koruyucu olacağı görüşü ile uyumludur. Diğer taraftan, karbonhidrattan zengin bir diyetin plazma adiponektin düzeylerini azalttığı bilinmektedir (40). Ayrıca, oksidatif stresin de adiponektin ekspresyonunu inhibe ettiği ileri sürülmüştür. Bu durumun, obezitede adipoz dokuda oksidatif stres artışı ile uyumlu olarak, plazma adiponektinindeki azalmayı açıklayabileceği düşünülmektedir (37).
Adiponektinin insülin direncini düzeltip düzeltemeyeceği, obeziteye bağlı metabolik sendrom ve tip 2 diabet geliştirilmiş bir fare modelinde araştırılmıştır (41). Yüksek yağlı diyetle beslenen farelerde plazma adiponektin düzeyleri azalmıştır. Adionektin verilmesi, yüksek yağlı diyetle indüklenen insülin direnci ve hipertrigliseridemiyi anlamlı olarak iyileştirmiştir. Bu veriler, yüksek yağlı diyetle indüklenen obezitenin yol açtığı adiponektin düzeyindeki azalmanın, insülin direnci ve metabolik sendrom ile nedensel olarak ilişkili olduğunu güçlü biçimde ortaya koyar.
Hastalarımızda hem stresin, hem de ilaçların etkisiyle iştah ve besin tercihinde değişiklik olduğu, özellikle yağlı ve şekerli besinlere ilgilerinin arttığı gözlenmektedir. Stres karşısında nöroendokrin sistemin aktive olduğu; epinefrin, norepinefrin, kortizol, büyüme hormonu ve glukagon sekresyonunun arttığı bilinmektedir (42). Bu nöroendokrin değişiklikler, plazma glikozunun regülasyonundan sorumlu bütün mekanizmalarla etkileşerek hiperglisemi oluşturur. Çalışmamızda psikiyatrik ilaç kullananlarda, anlamlı olmamakla birlikte, kortizol düzeyi daha yüksek bulunmuştur (Tablo-2). Ayrıca, glikoz, insülin, HbA1C ve HOMA-IR düzeylerinin de kontrol grubundan anlamlı olarak daha yüksek bulunmasında stresin nöroendokrin sistemi aktivasyonu ve bunun karbonhidrat metabolizmasına etkisinin rolü olabileceği düşünülmüştür. Bundan başka, çalışma grubunda psikiyatrik bozukluk nedeniyle ilaçla tedavi edilme süresi HbA1C ile anlamlı pozitif korelasyon göstermiştir (r=0.371, p=0.040). Bu sonuçlara göre, psikiyatrik bozukluk ve uygulanan tedavilerin hastaları DM, metabolik sendrom ve KVH yönünden daha duyarlı kıldığını gösterdiği düşünülmüştür.
Obezite, koroner arter hastalığının (KAH) gelişmesi için bağımsız bir risk faktörüdür (43). Obezite dolaylı olarak insülin direnci, hiperlipidemi ve hipertansiyon ile ilişkisi üzerinden de KAH riskini artırır. Karın içinde yağ birikimi (visseral adipozite) olumsuz koroner risk profili ile yüksek korelasyon gösterir. O nedenle, klinikte bel çevresi ve vücut yağı ölçülmesi önem kazanmıştır. Kadınlarda bel çevresinin 80 cm’in üzerinde olması metabolik sendrom ve KVH açısından risk artışına işaret etmektedir (44). Çalışmamızda, bel çevresi her iki grupta da bu değerin üzerinde olup, çalışma grubunda kontrol grubundan anlamlı olarak daha yüksektir. Ayrıca, kalça çevresi ve vücut yağ yüzdesi de çalışma grubunda anlamlı olarak daha yüksek bulunmuştur (Tablo-1). Kontrol grubunda ölçülen vücut yağ oranı, BKI’ye göre beklenen (45) düzeye uygun iken; çalışma grubunda beklenen değerin üzerinde olduğu bulunmuştur.
Tip 2 DM’li hastalarda metabolik sendromun varlığı kardiovasküler hastalık (KVH) açısından özellikle yüksek risk oluşturur (46). Metabolik sendromlu hastalarda fibrinojen ve PAI-1 gibi protrombotik faktörler genellikle yüksektir. Ayrıca, sitokinlere (TNF- ve IL-6 gibi) ek olarak, CRP ve fibrinojen gibi akut faz reaktanlarında da artış olur. CRP düzeyleri metabolik sendromlu hastalarda normal kişilere göre daha yüksek olma eğilimindedir (46). Yüksek CRP düzeyleri ( 3 mg/L) KVH için açık bir risk faktörü olarak kabul edilir (47). Çalışmamızda CRP ve fibrinojen düzeyleri her iki grupta da yüksek olup, CRP ilaç alanlarda laboratuarımızın verdiği referans aralığından (0-5 mg/L) daha yüksek bulunmuştur (Tablo-2). Diğer taraftan, çalışma grubunda adiponektin HDL (r=0.371, p=0.037) ve folat (r=0.430, p=0.014) ile anlamlı pozitif, fibrinojen (r= -0.464, p=0.015) ile anlamlı negatif korelasyon gösterirken; kontrol grubunda adiponektin ve fibrinojen arasında böyle bir ilişki saptanmamıştır (r= -0.100, p=0.667). Ayrıca, akut faz reaktanlarından ferritin, çalışma grubunda anlamlı olarak daha yüksektir (Tablo-2). Bel çevresi ve vücut yağındaki artışla birlikte inflamasyon göstergelerindeki bu artışlar, diğer biyokimyasal bulgularla birlikte, psikiyatrik ilaçlarla tedavi edilen hastaların metabolik sendrom ve KVH yönünden, psikiyatrik farmakoterapi almayan obezlere kıyasla daha fazla riskli olduklarını doğrular niteliktedir.
Sukroz (Suc) ile beslenen ratlarda antipsikotik chlorpromazine (Cp)’in vücut ağırlığı, glikoz, lipit metabolizması ve adipositokin sekresyonuna etkisin araştırıldığı bir çalışmada (48); adiponektin düzeyleri sadece sukroz alan grupta kontrol grubundan anlamlı olarak daha yüksek bulunurken, Suc+Cp grubunda değişmemiştir. Plazma TNF- düzeyleri Suc+Cp grubunda, sadece Suc alan gruptan anlamlı olarak daha yüksek bulunmuştur. Açlık glikoz ve insülin düzeyleri ise Suc ve Suc+Cp gruplarında, kontrol grubundan anlamlı olarak daha yüksek bulunmuştur. Bu sonuçlar, obezite ile uyumlu olarak plazma TNF- düzeylerindeki artışa adiponektinin kompansatuar cevabını Cp’in inhibe ettiği şeklinde yorumlanmıştır.
PON, başlıca HDL ile ilişkili bir enzimdir. Atherogenesis ve demansa yatkınlığı artıran lipit peroksidasyonunun önlenmesinde önemli fonksiyonel bir role sahiptir (49,50). Düşük serum PON-1 aktivitesinin olumsuz kardiovasküler olaylar yönünden yüksek riske işaret ettiği ve bunun demans gelişiminde etkili olduğu bildirilmiştir (51).
Pek çok çalışma obezlerde lipit ve lipoprotein metabolizmasında değişiklikler olduğunu göstermiştir. Hiperkolesterolemi, yüksek trigliserid (TG) ve LDL düzeyleri ile düşük HDL düzeyleri obez insanlarda sıklıkla gözlenmektedir. Lipoprotein düzeyleri ve kompozisyonundaki değişikliklerin, muhtemelen obezite ile ilişkili KVH yönünden daha fazla risk ile ilişkili olduğu düşünülmektedir. Bundan başka, obezlerde LDL’nin lipit peroksidasyonuna duyarlılığının daha yüksek oluşu ile oksidatif streste bir artış olduğu gösterilmiştir (9,10). Oksidatif hasardaki artışın, antioksidan kapasitedeki azalma ile uyumlu olabileceği öne sürülmüştür. Hayvan modellerinin kullanıldığı çalışmalarda, obezitede LDL oksidasyonunun HDL ile ilişkili enzimler olan paraoxanase (HDL-PON) ve lesitin kolesterol asiltransfarez aktivitesindeki azalma gibi HDL’nin antioksidan savunmalarındaki bozulma ile ilişkili olduğu gösterilmiştir (11).
HDL-PON, okside fosfolipitleri hidroliz edebilen kalsiyum bağımlı bir esterazdır ve böylece lipoproteinler (LDL, HDL) ve membranları oksidatif değişimlerden korur (12). PON-HDL’nin enzimatik aktivitesi sağlıklı insanlar arasında geniş değişkenlik gösterir ve düşük PON aktiviteli bireylerin oksidatif stres ve lipit peroksidasyonunun rol oynadığı hastalıkların gelişmesi yönünden daha fazla riskli oldukları bildirilmiştir (12). Bundan başka, HDL’nin antioksidan kapasitesi ve oksidasyon, glycation ve homosisteinilasyon gibi aterojenik modifikasyonlara duyarlılığının HDL-PON aktivitesi ile ilişkili olduğu önceki çalışmalarla gösterilmiştir (52,53).
Sağlıklı ve obez bireylerde HDL-PON aktivitesi ve lipoprotein oksidatif stres arasındaki ilişkinin araştırıldığı bir çalışmada (49), obezlerde HDL-PON aktivitesi kontrollere kıyasla anlamlı olarak daha düşük bulunmuştur. HDL-PON aktivitesi ve lipit peroksitlerin arasında negatif korelasyon bulunması, paraoksanaz aktivitesi ve lipoproteinlerin lipit peroksidasyonu arasındaki ilişkiyi doğruladığı şeklinde yorumlanmıştır. Plazma leptin düzeylerinin obezlerde HDL-PON aktivitesi ile negatif ve lipit hidroksi peroksitleri ile pozitif korelasyon göstermesi, leptin ile lipoproteinlerin oksidatif hasarı arasındaki ilişkiye bağlanmıştır. Daha düşük paraoksanaz aktivitesi ve HDL ile LDL’ deki kompozisyonel değişikliklerin obezite ile ilişkili KVH riskinin daha büyük olmasına katkıda bulunduğu sonucuna varılmıştır.
Bir çalışmada (54), PON/HDL oranı HOMA indeksi, BKİ ve BKO ile anlamlı pozitif korelasyon göstermiştir. Hiperglisemi olmayan bir toplulukta, insülin direnci veya hiperinsülineminin paraoksanaz aktivitesinin intragenotipindeki değişkenliğe katkıda bulunan bir faktör olduğu sonucuna varılmıştır.
Çalışmamızda, PON düzeyleri ve PON/HDL oranları ilaç alanlarda daha düşük olmakla birlikte, bu farklılık anlamlı bulunmamıştır (Tablo-2). Çalışma grubunda PON, BKO ile anlamlı pozitif korelasyon; PON/HDL ise TG ile anlamlı pozitif, HDL ile anlamlı negatif korelasyon göstermiştir. Kontrol grubunda böyle bir ilişki saptanmamıştır. Çalışma grubunun bu sonuçları, KVH riski artışına işaret eden diğer bulgularıyla uyumlu bulunmuştur.
Literatürde psikiyatrik bozukluğu nedeniyle farmakoterapi almakta olan hastalarda leptin ve adiponektin düzeylerinin araştırıldığı çalışmalar bulunmaktadır. Çalışmamız, zengin biyokimyasal parametrelerle birlikte HDL-PON aktivitesinin de çalışıldığı ilk çalışmadır.
Metabolik sendromun yüksek prevalansında obezite epidemisi önemli rol oynar. Abdominal obezite, glukoz, insülin ve lipid metabolizmasındaki bozukluklarla güçlü bir şekilde ilişkilidir (17-20, 55). Sendrom X, insülin direnci sendromu veya öldürücü dörtlü (deadly quarted) gibi terimler, metabolik sendromun hipertansiyon, hiperlipidemi ve glukoz tolerans bozukluğu (IGT) ile ilişkisini ifade etmek için kullanılmıştır. Bu kişilerde, kanda ürik asit, leptin, insülin, fibrinojen, CRP yüksekliği yanında, klinikte depresyon, non-alkolik steatoz ve polikistik over sendromuna daha sıklıkla rastlanmaktadır. Besin alımının artması, inaktivite ile birlikte vücutta visseral yağ dokusu artışına ve sonunda insülin direncine katkıda bulunur. Visseral yağ kitlesinin insülin direncinin boyutu ile pozitif korelasyon gösterdiği bilinmektedir (56).
Çalışmamızda ilaç tedavisi almakta olan hastaların antropometrik ve biyokimyasal tablolarındaki bozulmalara ek olarak; beslenmelerinde toplam kalori, doymuş yağ ve rafine karbonhidrat tüketimlerinin daha yüksek, buna karşılık kompleks karbonhidratlar, antioksidan vitaminler, esansiyel yağlar ve lif tüketimlerinin daha düşük olduğu alınan derinlemesine beslenme hikayesi ve besin tüketim kayıtlarından belirlenmiştir. Bu nedenle, psikiyatrik bozukluğu nedeniyle farmakoterapi başlanması gereken hastaların multidisipliner bir ekip yaklaşımı ile takip ve tedavisinin yapılmasının yararlı olacağı sonucuna varılmıştır.
Translation - English Obesity, which currently has a rapidly increasing prevalence, is closely associated with type-2 diabetes, cardiovascular diseases, and hypertension, and metabolic syndrome, in which these diseases coexist. In addition to energy storage, the adipose tissue also regulates the energy metabolism via secretion of adipokines into the circulation. Insulin resistance lies atop the factors providing the ground for obesity-related disorders (1). Stimulating cellular glucose uptake and acting as an antilipolytic hormone, insulin may cause weight gain via direct action on the adipose tissue and influencing appetite through hypoglycemia. Leptin, which is synthesized by adipocytes, regulates appetite and body weight via acting on leptin receptors in the satiety center in the hypothalamus. Administration of leptin reduces nutrient intake and body weight. However, it is believed that in obese people, there are increases in appetite and weight associated with leptin resistance in the hypothalamus. Also, it is known that insulin stimulates leptin synthesis in adipocytes, and conversely, leptin can have both stimulating and inhibiting effects on the insulin secreting function of pancreatic beta cells. The plasma leptin levels have prominent effects on the body mass index (BMI) and body fat ratio.
Adiponectin, a hormone with a significant role in regulation of sensitivity to insulin, is reduced in concentration in obesity and type 2 diabetes (2). Adiponectin is related both to lipid and glucose metabolisms and insulin resistance (3,4). Administration of recombinant adiponectin stimulates glucose uptake and lipid oxidation in muscles, while reducing lipid uptake and glucose synthesis in the liver and improving insulin resistance of the body as a whole (5). Other than these, adiponectin acts as an antiinflammatory factor, and contributes in reducing the risks of atherosclerosis, hypertension and coronary heart disease (6,7). Excess body fat, particularly abdominal visceral lipid deposition, is frequently accompanied by hypoadiponectinemia, a state which is a well-known mediator of the relationship between obesity and atherosclerotic vascular diseases (8).
Hypercholesterolemia, high triglyceride (TG) and LDL-cholesterol levels, and low HDL-cholesterol levels are frequently encountered characteristics of obese people. In various studies, it has been shown that obese people have increased oxidative stress, and greater sensitivity to lipid peroxidation of LDL as compared with healthy controls (9,10). Increased oxidative damage has been proposed to be associated with reduced antioxidant capacity. In obesity, LDL oxidation has been found to be related to disrupted function of HDL in antioxidant defense, in which reductions in the activities of HDL-associated enzymes such as paraoxanase (HDL-PON) and lecithin-cholesterol acyl transferase have been held responsible (11). The enzymatic activity of HDL-PON varies greatly among healthy people. The risk of diseases related to oxidative damage and lipid peroxidation in people with low PON activity has been suggested to be greater as compared with that in people with high activity (12).
During pharmacotherapy of psychiatric diseases, an increase occurs in body weight, frequently accompanied by increased appetite. This side effect, the occurrence and time of which is difficult to estimate, eventually leads to obesity, causing cancellation of treatment in a portion of patients despite therapeutic success. Obesity also increases the development risk of various chronic diseases. Obese patients have greater risks of coronary artery disease, hypertension, hyperlipidemia, diabetes, various cancer types, cerebrovascular diseases, osteoarthritis, pulmonary diseases, and sleep apnea (13-15). Therefore, weight reduction plays a key role in protection from several diseases or improving prognosis in an already developed disease. The purpose of this study was to investigate the relationship of body weight, composition and biochemistry as well as alterations in leptin, adiponectin and paraoxonase levels with metabolic and cardiovascular risk factors in adult females undergoing treatment with psychiatric drugs.
DISCUSSION
During treatment courses of various diseases, increasing body weight is regarded as a sign of improvement. However, the alterations induced by drugs on the mechanisms regulating appetite in the body may result in excessive weight gain, and therefore endanger compliance to medical treatment. Various drugs, as a negative outcome of their therapeutic effects, have the capacity to alter body weight (16). These include psychotropic drugs such as antipsychotics, antidepressants, and mood stabilizers.
Excessive fat deposition in the adipose tissue and abdominal distribution of this fat negatively influence health in humans. Abdominal obesity is strongly related to disorders in glucose, insulin, and lipid metabolisms (17-20). In addition to the role of energy storage, the adipose tissue also secretes various biologically active adipokines. Among these, leptin and adiponectin are known to have significant roles in insulin sensitivity.
Leptin, a protein synthesized not only by the adipose tissue but also by the hypothalamus, hypophysis, placenta, skeletal muscles, stomach, and milk glands, is particularly related to energy homeostasis and satiety (2). Circulating leptin levels increase paralleling the fat mass, and carry information to the hypothalamus about the amount of energy stored in the adipose tissue. Rare genetic mutations cause leptin or leptin-receptor deficiencies in humans. Administration of leptin to children who are extremely obese due to leptin deficiency results in significant reduction in their energy intake and fat mass (21). However, the failure of exogenous leptin administration to obese people in reducing adiposity reflects increased leptin content of adipose tissue and a state of leptin resistance against the high circulating leptin levels. It is thought that the leptin resistance in obese people is related to a disorder in transport of leptin to the brain and/or reduced hypothalamic leptin signals (2).
A common side effect of various psychiatric drug therapies, weight gain is thought to be contributed by reduced physical activity caused by the sedative effects of drugs, and excessive consumption of high-calorie drinks. Insulin and leptin play role in regulation of body weight. In one study (22), in which the levels of these hormones have been investigated in patients treated with various antipsychotics, patients receiving olanzapine have been found to have higher insulin levels than those being treated with conventional antipsychotics, despite similar BMI values. This situation was attributed to possible effects of olanzapine on insulin secretion. It has also been found that leptin regulation also changed during olanzapine and clozapine therapies, and the effects of these drugs on insulin and leptin levels were proposed to be associated with weight gain-inducing properties.
In our study, the patients being treated with various psychotropic drugs were found to have significantly higher glucose, insulin, HOMA-R, leptin, triglyceride, total cholesterol and LDL cholesterol levels than the controls, despite being within the same BMI group (BMI>30.00 kg/m2). Also, the body fat mass (%) and waist and hip circumference measurements were significantly higher in the patients on drugs. The significantly higher hepatic enzyme levels in the study group than the controls has been attributed not only to the disorders caused by obesity, but also to the drugs used. This situation indicates that psychotropic drugs may provide grounds for development of metabolic and cardiovascular disorders via affecting hormones related to body weight.
Weight gain caused by psychotropic drug treatment has been proposed to be associated with increased leptin levels. During acute treatment of schizophrenia with clozapine, serum leptin has increased twofolds by 2 weeks following the initiation of therapy, and remained high for the first 10 weeks. The body weight increased by a mean of 42 kg within this period (23). Patients treated with clozapine or olanzapine for 4 weeks were found to have gained mean weights of 2.3 and 3.9 kg, respectively, compatible with increased leptin secretion. On the other hand, patients treated with haloperidol or received no psychotropic drugs gained no weight and had stable leptin levels (24).
Metabolic syndrome has been reported to be considerably common among patients treated for schizophrenia (25). This situation indicates a significant increase in the risk of cardiovascular and metabolic disorders. Therefore, it has been stressed that initial evaluation and follow-up regarding the presence of metabolic syndrome-related risks should be a part of the clinical treatment in patients receiving antipsychotics.
In a longitudinal study (26), a mean increment of 3.3 kg in body weight was observed associated with a significant increase in plasma leptin following the 8th week of acute olanzapine therapy. Within the 8th week of lithium monotherapy, body weights of 8 female and 7 male inpatients increased by a mean of 5.9 kg, while the leptin levels have increased significantly from a basal value of 6.9 ng/ml to 10.3 ng/ml within this period (27). In epileptic patients on long-term valproic acid therapy, weight gain corresponding to increased BMI has been found to be associated with increased plasma leptin levels (28).
During 6 weeks of treatment with tricyclic antidepressants and paroxetine, body weight has been found to increase by 3.5 and 1.0 kg, respectively, while no change was observed in depressed patients not receiving pharmacotherapy. On the other hand, plasma leptin levels did not change in ay of the groups (29). In one study, a very minor increase as compared with the basal values was found in plasma leptin levels of 11 patients who gained a mean weight of 2.4 kg by the 4th week of mirtazapine treatment for major depression (in Psychiatric Drugs-1, 41).
In a comparative study in which the effects of atypical antipsychotics, namely quetiapine, olanzapine, risperidone, and clozapine on leptin and triglyceride levels and body weight were investigated (30), olanzapine and clozapine were found to cause significant increases in leptin and triglyceride levels, as well as in body weight. The increases in these parameters were moderate with quetiapine, and minimal with risperidone. Also, leptin was determined to correlate positively with BMI and triglyceride levels.
In our study, leptin had significant positive correlations with weight, BMI, and waist and hip circumferences in both groups, and a significant negative correlation with the adiponectin/leptin ratio.
Parameters with roles in glucose and lipid metabolisms and in nutritional control have been investigated in schizophrenia patients on long-term (0.5-8.3 years; mean: 2.5±1.9 years) risperidone monotherapy (31). The body fat percentage, BMI, and fasting blood glucose, triglyceride, leptin, ghrelin, and prolactin levels were found to be significantly higher in patients on risperidone therapy than the healthy controls, while the HDL-cholesterol levels were significantly lower. It was concluded that high plasma ghrelin levels might stimulate appetite and therefore accentuate the metabolic disruption.
The blood concentrations of adiponectin, which acts as a hormone increasing insulin sensitivity, are reduced in obesity and type-2 diabetes (2). Administration of recombinant adiponectin to rodents increases glucose uptake and lipid oxidation in muscles, while reducing fatty acid uptake and glucose production in the liver, and thus improves insulin resistance of the whole body (5). In humans, adiponectin is known to correlate negatively with body weight, body fat mass, and insulin levels (2).
Other than its associations with lipid and glucose metabolisms and insulin resistance, adiponectin acts as an antiinflammatory agent (3,32,33), and might contribute in development of obesity-related disorders such as atherosclerosis, hypertension and coronary heart disease (6,33,34). Excess body fat, particularly abdominal visceral fat deposition, is frequently associated with hypoadiponectinemia, a known mediator of the association between obesity and atherosclerotic vascular disorders (8).
In a study in which plasma adiponectin, leptin and resistin levels and the association of these adipokines with insulin resistance were investigated in obese and non-obese adults (35), insulin resistance correlated very strongly with leptin levels. Though the adiponectin levels were significantly lower in the obese than non-obese individuals, they were not found to be significantly correlated with HOMA-R.
In our study, the mean adiponectin levels of the group using psychotropic drugs were significantly lower than those of the controls (Table 2). Though the patients included in the study were obese regarding their BMI (>30 kg/m2), there was a significant difference between groups regarding BMI values. Therefore, the groups were reevaluated following adjustments of some of the parameters to age and BMI. Following adjustment to an age of 38.67 years and a BMI of 33.54 kg/m2, mean adiponectin levels were found to be 5.838±0.709 g/ml in the group on drugs, and 8.8040.877 g/ml in the controls, the difference being statistically significant (p=0.017).
Plasma adiponectin and leptin levels, body composition, and glucose consumption were investigated in adult females varying with regard to age and obesity level (36). In that study, plasma adiponectin levels were found not to vary with age, but to be negatively associated with body fat percentage, visceral fat, subcutaneous abdominal fat, and insulin and leptin levels. Besides, adiponectin was found to be positively correlated with glucose consumption.
In our study, the adiponectin levels were found to have a significant positive correlation with age in patients on psychiatric drugs (r=0.525; p=0.002). Besides, adiponectin had significant positive correlations with glucose, HDL, and folate levels, and adiponectin/leptin ratio and a significant negative correlation with fibrinogen in patients on drugs, while it had significant positive correlations with PON and vitamin B12 levels, and adiponectin/leptin ratio in the controls. These results were thought to confirm the association of adiponectin with cardiovascular diseases.
Hypoadiponectinemia has been reported to be more strongly correlated with diabetes and metabolic syndrome than the other inflammatory indicators (37). Reduced plasma adiponectin levels are also encountered in states frequently associated with insulin resistance such as cardiovascular diseases and hypertension (38,39).
The plasma adiponectin levels are affected by various factors, including sex, aging and lifestyle. Several dietary factors such as soya proteins, cod liver oil, and linoleic acid have been proposed to increase plasma adiponectin levels (37). This situation is compatible with the view that ingestion of these factors is protective against the development of diabetes. On the other hand, carbohydrate-rich diets are known to reduce plasma adiponectin levels (40). Besides, oxidative stress has also been proposed to inhibit the expression of adiponectin. This situation is regarded as a potential explanatory factor for the reduced plasma adiponectin levels compatible with the increased oxidative stress in the adipose tissue in obesity (37).
Whether adiponectin can improve insulin resistance has been investigated in a murine model in which obesity-related metabolic syndrome and type-2 diabetes have been developed (41). Mice fed with high-fat diet had reduced plasma adiponectin levels, and administration of adiponectin significantly improved insulin resistance and hypertriglyceridemia induced by high-fat diet. These data strongly demonstrate that reduced adiponectin levels as a consequence of high-fat diet-induced obesity have a causal relationship with insulin resistance and metabolic sydrome.
It has been observed that both stress and drugs have had effects on appetite and dietary choices in our patients, who particularly had increased tendencies towards fatty and sweet nutrients. It is known that the neuroendocrine system is activated by stress, with increasing epinephrine, norepinephrine, cortisol, growth hormone, and glucagon secretions (42). These neuroendocrine alterations lead to hyperglycemia via interacting with each mechanism responsible for the regulation of plasma glucose. In our study, the cortisol levels, though the difference was not statistically significant, were found to be higher in the group on psychiatric drugs (Table 2). Besides, it has been proposed that the stress-induced activation of neuroendocrin system and its effects on the carbohydrate metabolism might be responsible for the significantly higher glucose, insulin, HbA1c and HOMA-IR levels in the patients on drugs than the controls. The duration of drug therapy also had a significant positive correlation with HbA1c levels in the psychiatric drug receiving group (r=0,371, p=0,040). These results indicate that psychiatric disorders and corresponding treatments render the patients more susceptible to DM, metabolic syndrome, and cardiovascular diseases.
Obesity is an independent risk factor for development of coronary artery disease (43), and also increases the risk of coronary artery disease indirectly via its associations with insulin resistance, hyperlipidemia, and hypertension. Visceral adiposity, which is the fat deposition within the abdomen, has a high correlation with unfavorable coronary risk profile. Therefore, measurements of waist circumference and body fat have gained significance in the clinics. A waist circumference greater than 80 cm in females is indicative of increased risks of metabolic syndrome and cardiovascular diseases (44). In our study, the waist circumference was greater than that limit in both groups, being also significantly higher in the psychiatric drug group than the controls. Besides, the hip circumference and body fat percentage were significance higher in the psychiatric drug group (Table 1). While the body fat ratio was compliant with the level expected according to BMI in the controls (45), it was above the expected value in the group receiving psychiatric drugs.
The presence of metabolic syndrome in patients with type 2 diabetes mellitus particularly induces a high risk for cardiovascular diseases (46). In patients with metabolic syndrome, prothrombotic factors such as fibrinogen and PAI-1 are usually high. Besides, acute phase reactants such as CRP and fibrinogen are increased in addition to cytokines (e.g., TNF- and IL-6). The CRP levels tend to be higher in patients with metabolic syndrome than healthy individuals (46). A high CRP level (3 mg/L) is considered as an overt risk factor for cardiovascular diseases (47). In our study, both CRP and fibrinogen levels were high in either group, while the CRP levels were greater than the reference range of our laboratory (0-5 mg/L) in the group receiving drugs (Table 2). On the other hand, in the group receiving psychiatric drugs, adiponectin had significant positive correlations with HDL (r=0,371, p=0.037) and folate (r=0.430, p=0.014), and a significant negative correlation with fibrinogen (r=-0.464, p=0.015), while no such correlation of adiponectin with fibrinogen was observed in the controls (r=-0.100, p=0.667). Besides, ferritin, an acute phase reactant, was significantly higher in the patients on drugs (Table 2). These increases in indicators of inflammation paralleling increased waist circumference and body fat content, along with the other biochemical findings, support the hypothesis that patients on psychiatric drugs have higher risks of metabolic syndrome and cardiovascular diseases than obese individuals who do not receive psychiatric pharmacotherapy.
In a study in which the effects of antipsychotic chlorpromazine on body weight, glucose and lipid metabolism, and adipocytokine secretion were investigated in rats fed with sucrose (48), the adiponectin levels were significantly higher in the group receiving sucrose only than the controls, while they were not altered in the group receiving sucrose and chlorpromazine. On the other hand, plasma TNF- levels were significantly greater in the group receiving sucrose and chlorpormazine than those receiving sucrose only. Both the group receiving sucrose and the group receiving sucrose and chlorpromazine had significantly higher fasting glucose and insulin levels than the controls. These results were attributed to that, compatible with obesity, the compensatory response of adiponectin to the increase in plasma TNF- levels is inhibited by chlorpromazine.
PON is an enzyme particularly associated with HDL., and has a significant functional role in prevention of lipid peroxidation, which increases the tendency towards atherogenesis and dementia (49,50). It has been reported that low serum PON-1 activity indicates high risk of unfavorable cardiovascular events, and this is effective in development of dementia (51).
Alterations in lipid and lipoprotein metabolisms in obese individuals have been demonstrated in several studies. Hypercholesterolemia, high triglyceride and LDL levels, and low HDL levels are frequently encountered in obese people. It is thought that the alterations in lipoprotein levels and composition are possibly associated with higher obesity-related risk of cardiovascular diseases. Also, increased oxidative stress paralleling increased sensitivity of LDL to lipid peroxidation has been shown in obese individuals (9,10). The increased oxidative damage has been proposed to be possibly related to reduced antioxidant capacity. In studies in which animal models have been used, LDL oxidation in obesity has been found to be associated with disruption in antioxidant defence mechanisms of HDL, including reduced activities of HDL-associated enzymes such as paraoxanase (HDL-PON) and lecithin cholesterol acyltransferase (11).
HDL-PON is a calcium-dependent esterase that can hydrolyze oxidized phospholipids, therefore protecting lipoproteins (LDL, HDL) and membranes from oxidative changes (12). The enzymatic activity of PON-HDL varies greatly among healthy people, and individuals with low PON activity have been reported to be under greater risk of developing diseases in which oxidative stress and lipid peroxidation play role (12). Besides, it has been shown in previous studies that the antioxidant capacity of HDL and its sensitivity to atherogenic modifications such as oxidation, glycation, and homocysteinization are associated with HDL-PON activity (52,53).
In a study in which the relationship between HDL-PON activity and oxidative stress of lipoproteins in healthy and obese individuals has been investigated (49), the HDL-PON activity has been found to be significantly lower in obese people than the controls. The presence of a negative correlation between HDL-PON activity and lipid peroxides has been regarded to confirm the association between paraoxonase activity and lipid peroxidation of lipoproteins. The finding that plasma leptin levels correlated negatively with HDL-PON activity and positively with lipid hydroxy peroxides in obese individuals has been attributed to the relationship between leptin and oxidative damage of lipoproteins. It has been concluded that the lower paraoxonase activity and compositional alterations in HDL and LDL contribute to the increased risk of obesity-related cardiovascular diseases.
In one study (54), the PON/HDL ratio has been found to have significant positive correlations with the HOMA index, BMI, and WHR. It has been concluded that, in a population without hyperglycemia, insulin resistance or hyperinsulinemia are factors contributing the variability in the intragenotype of paraoxanase activity.
In our study, although the PON levels and PON/HDL ratios were lower in the patients on drugs, the difference was not statistically significant (Table 2). In the group of patients on drugs, PON had a significant positive correlation with BMI, while PON/HDL ratio had a significant positive correlation with triglyceride and a significant negative correlation with HDL levels. No such association was observed in the controls. These results in the patients on drugs have been found to be compatible with the other findings indicating increased cardiovascular disease risk.
Studies in which the leptin and adiponectin levels have been investigated in patients receiving pharmacotherapy for psychiatric disorders have been reported in the literature. Our study is the first in which the HDL-PON activity also has been assessed along with a wide spectrum of biochemical parameters.
The epidemiology of obesity plays a significant role in the high prevalence of metabolic syndrome. Abdominal obesity is strongly related to disorders of glucose, insulin and lipid metabolisms (17-20, 55). Terms such as syndrome X, insulin resistance syndrome, or deadly quartet have been used for expressing the association of metabolic syndrome with hypertension, hyperlipidemia, and impaired glucose tolerance. In these individuals, not only high blood uric acid, leptin, insulin, fibrinogen and CRP levels, but also clinical depression, non-alcoholic steatosis, and polycystic ovary syndrome are more commonly encountered. Increased nutrient intake, together with inactivity, contributes to increased visceral fat tissue, eventually leading to insulin resistance. The visceral fat mass is known to correlate positively with the severity of insulin resistance (56).
In our study, in addition to the deteriorations in anthropometric and biochemical profiles of the patients on psychiatric drugs, it has been learned from the detailed anamnesis and nutritional records that the nutritional profiles of these patients included increased total caloric intake and higher saturated fat and refined carbohydrate content versus lower complex carbohydrate, antioxidant vitamin, essential fat and fiber consumption. Therefore, it has been concluded that patients candidate for pharmacotherapy of psychiatric disorders should be treated and followed up with a multidisciplinary team approach.

Sistemin ihtiyaç duyduğu sekonder frekans kontrol yedeği, primer frekans kontrol yedeğinin serbest kalmasını sağlayacak miktarda TEİAŞ tarafından belirlenir. Sekonder frekans kontrol yedeğinin bu ihtiyacı karşılamakta yetersiz kalması durumunda ilave olarak tersiyer kontrol yedeği kullanılabilir. Santral/blok/ünite sekonder frekans kontrol maksimum kapasite (MAXC) ve sekonder frekans kontrol minimum kapasite (MINC) değerlerinin, ünite bazında elle girilen minimum ve maksimum limit değerleri kullanılarak ünitelerin çalışma konumları (AUTO/MAN) dikkate alınarak doğru bir şekilde hesaplanıp, bu değerler ve ünitelerin aktif güç çıkışları ölçümlerinin doğru bir şekilde ve 2 saniyede bir TEİAŞ Yük Tevzi Merkezine gönderildiği kontrol edilecektir. Translation - English The secondary frequency control backup is the portion of the operational backup that is used by way of the automatic production control program and chosen in a way to be sufficient for the process of releasing the primary frequency control backup that is used for frequency control, restoration of the frequency to its nominal value, and maintenance of the total electrical energy exchanges with the neighboring electricity networks at the programmed level. The secondary frequency control backup required by the system shall be determined by TEİAŞ at a quantity to ensure that the primary frequency control backup is released. In case the secondary frequency control backup fails to meet this requirement, a tertiary control backup can be used in addition. The secondary frequency control backup should be continuously provided in order to be used both in cases of deviations that can be seen under normal operating conditions and imbalances between production and consumption due to major malfunctions.
In case the monitoring and controls display that the unit or block participating in the balancing system has failed in fulfilling the commitments within the framework of the principles of connection and balancing, TEİAŞ shall warn the user with a written notice. The user should inform TEİAŞ as soon as possible about the measures to be taken to overcome the causes of failure. TEİAŞ and the user negotiate the details of these measures. In case new operational stipulations are to be applied to the unit or block participating in the balancing system, TEİAŞ may require conduction of a test on the unit or block upon failure to reach an agreement within 10 days with regard to these stipulations and the date of their commission.
The speed gradient and dead band values of the units must be adjustable. The speed gradient of the unit should be adjusted to provide the maximum primary frequency control backup capacity determined by the primary frequency control performance tests. The primary frequency control backup quantity that the unit will provide must be provided with a limiting or similar function towards loading/damping. The dead band of speed control system of units should be adjustable to 0 (zero) when necessary. In case the values of speed gradient and dead band are required to have different values by TEİAŞ according to the requirements of the system, these values should be adjusted as specified by TEİAŞ.
Before the secondary frequency control tests, the power plant/block/unit should be included in the SCADA system of TEİAŞ, with the design document of the system/interface that should be implemented on the power plant for the purpose of administration of the power plant to the secondary frequency control submitted to TEİAŞ for approval, and this system must be fully compliant with the requirements of Automated Generation Control (AGC) program in the NLDC of TEİAŞ according to the design document approved by TEİAŞ. During these tests, the functional specifications of the Automated Production Control System/Interface installed in the power plant and human machine interface (such as screenshots) along with the participation performance of the power plant in the secondary frequency control via adjustment values sent through the SCADA system by the AGC in the NLDC shall be tested.
Whether the secondary frequency control maximum capacity (MAXC) and secondary frequency control minimum capacity (MINC) values of the power plant/block/unit, are precisely calculated using the manually-entered minimum and maximum limit values on the unit scale, taking the working conditions (AUTO/MAN) of the units into consideration, and whether these values and the active power output measurements of the units are correctly reported once 2 seconds to the Load Dispatch Center in TEİAŞ shall be checked. For the Combined-Cycle Gas Turbine Blocks, detailed examination shall be made on how the limits of the steam turbines producing electricity in connection with the gas turbines are calculated, and whether their per block secondary frequency control maximum capacity (MAXC) and per block secondary frequency control minimum capacity (MINC) values are correct.
In case the units do not participate in the primary frequency control: In the situation in which the units do not participate in the primary frequency control, thus without a primary frequency control backup, the MAXC and MINC values of the power plant shall be adjusted maximize the range of the secondary frequency control and the AGC program at the NLDC shall send the adjustment values for loading/damping instructions within the secondary frequency control range to the power plant/block/unit in the TEST MODE to check whether the power plant/block/unit respond within the whole range of secondary frequency control at the desired response rate, followed by the necessary adjustments in the power plant to obtain the desired secondary frequency control response. In this test, whether the power plant/block/unit maintains the secondary frequency control response at the desired final production level in a stable manner shall also be controlled.
In case the units participate in the primary frequency control: The secondary frequency control response shall thus be tested during contribution of the units to the primary frequency control. In this case, the secondary frequency control limits of the power plant, and therefore secondary frequency control maximum (MAXC) and minimum (MINC) values of the power plant/block/unit shall be adjusted in a way to cover the primary frequency control backups of the units. During these tests, the AGC program in NLDC shall send adjustment values in TEST MODE as loading/damping instructions to the power plant/block/unit within the secondary frequency control range in order to check whether the power plant/block/unit responds within the whole range of the secondary frequency control range at the desired rate, followed by adjustments until the desired secondary frequency control response is obtained. During these tests, whether the power plant/block/unit can give the desired response within the secondary frequency control range as well as a suitable primary frequency control response in the range of the primary reserve backup, in compliance with the droop settings of the speed governor of the unit, depending on the alterations in the system frequency, while participating in the secondary frequency control of the power plant/block/unit, and maintain this response shall be examined.
In order to determine the sensitivity of the unit, a quantity of frequency deviation is applied to the input of the speed governor of the turbine while preventing data acquisition from the mains network, starting with f=-5 mHz and changing on the positive and negative directions at stepwise increments of 5 mHz until a unit response is observed. Vane movement and/or changes in other related signals are the criteria for unit response. A frequency deviation of f=-5 mHz or simulated frequency value of f=49.995 Hz is applied in a stepwise manner as shown in Figure E.4.A.6 and kept in this state for at least one minute. At the end of this period, the nominal frequency value of 50 Hz is restored and after the unit reaches a stable state of the same Pset value, the process is repeated with a frequency deviation of f=+5 mHz or simulated frequency of f=50.005 Hz. In case the unit does not respond to frequency deviations of ±5mHz, the same procedures are repeated for frequency deviations of ±10mHz.
Article 126/G- When considered necessary in light of the technical studies that are conducted by TEİAŞ, the capacities of units added to the present production plants and/or of the new production plants may be leased via tender agreements made with the approval of the Ministerial Cabinet under the provisions of Electricity Market Ancillary Services Regulation. The probability of failure to meet the peak load for one year on a regional basis calculated in technical studies carried out by TEİAŞ shall be compared to the probability of failure to meet the peak load described in Article 43 of this Regulation. The requirement of regional capacity leasing shall be determined in regions where the probability of failure to meet the peak load estimated by TEİAŞ exceeds the targeted value described in Article 43.
