To present a request to the civil status Officer (1*), to provide the necessary informations : If the death took place in a hospital (2*): to sign the 2 specimens of the register of deaths inscription by the informant.
- For those which died outside the national territory, the declarant must do the so-called stages at the consulate or of the embassy or the diplomatic representation of Tunisia.
- Observations : (1*) the declarent is a relative of deceased or any person having exact and complete informations (more that possible). (2*) to be satisfied with the statement made by the hospital in order to avoid a double inscription of the death.
