It may stands for 'Registrado na Conservatória, which may reference to what Paulinho already said, but it could also stands for 'Registrado na Consulta' (registered at the appointment) and the internal number of the medical appointment.

I do not have more context unfortunately other data in the medical report are related to the patient. That part is at the end of the document. Thank you all for your help.
