I do agree that "discharge recommendations" will do the trick. However, "post-discharge" is an oft-used term as well.

Szczerze powiem, że nie widziałem terminu post-discharge. Z reguły jest samo recommendations, ewentualnie discharge recommendations. Mówię o kartach wypisowych z UK/US/CAN, ale nie twierdzę, że tak musi być, nie wszystko człowiek widział i może dobrze, dzięki temu życie jest ciekawsze. Pozdrawiam
