I understand that the scope and application of this form covers a broader base than food alone but the question is dealing with the title of the relevant form and stylistically I feel "etc" to be bad English. The convention already exists where "等" is not translated by JETRO in English language literature relating to the importation of food, packaging for food, materials used in food manufacture, and toys. Including all the items covered by the form in the name would result in an overly cumbersome and unnecessarily long name.

Obviously the foodstuff is packaged for transportation. Using etc. or "other articles" renders the translation too vague and too broad and may cause confusion. Saying food and associated packaging is redundant in this context.

通常は、私も役人の使う「等」は、訳さないことが多いですが、「食品」に容器包装は、含まれません。

Another option is "Certificate of Notification for Imported Food" but I would leave out the etc. as this sounds unnatural.

certificate of import notification of foods, etc

長ったらしいですが食品衛生法の表記に忠実に訳すとこうなると思います。

4 輸入者が別表第十二の中欄に掲げる食品、添加物、器具又は容器包装(以下この条において「食品等」という。)を輸入した場合において、当該食品等と同一の製品又はこれに準ずるもの(以下「同一食品等」という。)の同表の下欄に掲げる期間における輸入計画(当該期間に予定する輸入に係る貨物の積込重量、積卸港及び到着年月をいう。以下同じ。)を記載した輸入届出書の提出を行つているときは、当該期間に行おうとする同一食品等の輸入については、第一項本文の規定にかかわらず、当該提出をもつて同項の輸入届出書の提出に代えることができる。ただし、当該輸入に係る食品等が次の各号のいずれかに該当し、又はそのおそれがあるときは、この限りでない。
(4) When an importer imports the food, additives, apparatus, containers and packaging listed in the middle columns of appended table 12 (hereinafter referred to as "food, etc." in this Article) and when he/she has submitted a written import notification containing the import plans for the same products as said food or the equivalent thereto (hereinafter referred to as the "same food, etc.") for a period listed in the right hand columns of the same table (such plans shall mean the loaded weight, unloading port, and arrival date of the cargoes pertaining to the import planned for said period; the same shall apply hereinafter), said submission may, notwithstanding the provisions of the main clause of paragraph (1), replace the submission of a written import notification under paragraph (1) with regard to the import of the same food, etc. planned for said period; provided, however, that this shall not apply to the case where the food, etc. pertaining to said import falls under or is suspected of falling under any of the following items:

neutral David Gibney : The "Notification Form for Importation of Foods, etc" is a translation of the Japanese form and wasn't done by a native English speaker as it contains grammatical errors. It's a different form, translated poorly. Why repeat mistakes?
