careful monitoring and support services, where services are both medical and social support, where the patient is treated and cared for in the their own home with community services: psychiatric field social worker, community nurse, contact from relevant voluntary groups, etc. (my personal experience of providing these services was a long long time ago, but I don't think the terminology has changed a lot - except for bi-polar of course which is a new term for a very old disorder, which hasn't changed a jot).
Clearly "attivazione" is superfluous and would be wordy in Engliish, nobody provided "inactive services".
