In tunnelling and underground work this is the design of a tunnel or underground construction during construction itself. When driving a tunnel through well-surveyed solid good qualit rock, as in the case of the Channel Tunnel, this isn't necessary. If however you are driving through poor quality ground, loose soil. clay, water logged ground, either very close to the surface in a city or very deep where the ground can "squeeze" and even crush a TBM, then the final design decisions are taken as the tunnel is driven.

This interpretation is appropriate if you have a dam where the foundations require a lot of underground work often with tunnelling into hill or mountain slopes and the "design" is fine-tuned during construction.
Otherwise if everything is designed on the drawing board beforehand (as is normally the case with surface construction) with the help of geologists then Lindsay's answer is the right one.
