The term infarction is OK, but if the translation is for US, it is called "heart attack" by preference.

massive myocardial infarct
Again technically the other terms are not wrong. However in the hospital setting we do not regularly use the terms "extensive infarction" or "infarctus" prefering massive infarct or infarction. massive heart attack is okay translating for a lay audience.
