Pour mention may be in the same family of expressions as "Pour valoir ce que de droit". They are expressions used with documents that have a certain value (attestation of certificate, etc.)

"For insertion" sounds more reasonable and "Rectified" might also work, I suppose, depending on how I reformulate.
