Confirmation of continuing validity of birth certificate?
Title of official document issued by Moroccan municipal council to confirm birth certificate details and continuing existence of an individual at a certain date after his/her birth for administrative purposes.

Sorry my answer is a bit late, but I think you need to be careful with "certificate of existence" - it is used much more frequently for businesses than for individuals. See examples:

A "Life Certificate" does not just exist in Morocco. Many examples on the web, meaning a certificate that someone is alive:

I don't think it is necessary to say 'individual' or 'personal', but that's up to you.
The individual record sheet that Nicholas is referring to is probably a "fiche individuelle d'état civil" (which no longer exists in France - Morocco's administration is similar) or an 'extrait d'acte de naissance' and the "family record book" is certainly the "livret de famille". I live in France, am married to a Frenchman, so I have a 'livret de famille' and am familiar with the various certificates, although I must confess I had never heard of a 'certificat de vie'. It figures in the following list as the Spanish equivalent of an 'extrait d'acte de naissance' :

This type of document does not have an equivalent in English-speaking countries.

- Birth certificates are issued to Canada born applicants only. The Consulate will issue an Individual record sheet (instead of the birth certificate) for other applicants
This "individual record sheet" held by a person in place of his birth certificate is distinguished from the "family record book" that is given to a couple/family.

While in general "certificate of existence" in the Americas refers to corporations being in good standing, I was also able to find a single reference to "certificate of existence" in the context you refer to. I agree that this would probably be the better and more precise concept translation for this fairly unusual concept.
