When twins are born, one of them has to come out after the other, and that one is called the second born. That is what this means.

The "presentante" is the person who reports the birth for registration. In the UK and Ireland this person is called the informant. I'm not sure of the right term in the US, but we had "applicant" in a previous question, and a lot of people agreed:
